The following application can be used for either the Two Year Training or the Advanced Practitioner Program.

U.S. Applicants
Below is a link to the application found on google drive. Please download the form and PRINT the application/essays and SNAIL MAIL them to the address below.

Please include a $60 USD processing fee (affiliate code holders receive $20 off their application fee!). Cash or check are welcome. (written to Sarah Bly)
International Applicants

Once completed, please either:
Print and mail the application and application fee via post (as instructed above)
OR: Submit the application as a PDF to me DIGITALLY using the form below
OR (preferred if possible): Upload the application to Google Drive and send me the link via email to submit the application (sarah@thewellschoolofbodyliteracy.com)
